Passport for Elementary School Teachers
The 80-hour Follow-Up (Passport) Program is a district-sponsored activity. Contact your district representative for information related to the submission of Passports.
A Passport is being provided here for organizational purposes only should your district require submission.
- Download and print the 80-hour Follow-up (Passport) within two years of the completion of the initial professional development institute depending on your district requirements.
- Consult your district for passport requirements.
- Date and have initialed entries for 80 or more hours of district Passport activities.
- Include Passport event documentation, if required by the district. Consult with your site administrator or coach for district guidelines regarding documentation.
- Turn in the Passport to the site administrator or designee for certification per the district’s requirements.
